A 39-year-old man accused of murdering a police sergeant has appeared before the Tiyani Magistrate’s Court in Limpopo.

The accused, Awelani Kennedy Matumba, was remanded in custody, with the matter postponed to Tuesday, 14 April 2026, for a formal bail application and further police investigations.

According to police, the incident stems from a shocking double murder that took place at Nkuna Kraal Mall in Nkuzana village on Saturday, 09 August 2025, at approximately 15:00.

It is alleged that the female victim, a police sergeant, was out with two adults and her six-year-old child when her estranged husband — from whom she had been separated for about seven months — approached her.

A heated argument reportedly broke out between the couple.

Police say the suspect allegedly took a service firearm and shot the female sergeant in the upper body.

In a dramatic turn of events, as the suspect attempted to flee the scene, members of the community who witnessed the incident chased him down, assaulted him, and set him alight next to his vehicle.

The vehicle was completely burnt in the process.

Police confirmed that murder cases were opened following the incident.

Two additional suspects, aged 31 and 35, were arrested on 20 and 26 August 2025 in connection with the case.

Matumba, believed to be an accomplice, was arrested on Tuesday, 07 April 2026, at a hideout within the policing area.

The deceased have been identified as Sergeant Mavhetha Ottaniel, attached to the Thohoyandou K9 Unit, and Sergeant Mavhetha Rembuluwani, who was stationed at the Beit Bridge Port of Entry.

Police have appealed to the public for assistance in locating any remaining suspects.

Anyone with information is urged to contact Detective Sergeant Stanley Chauke on 072 554 3141, visit their nearest police station, or call the Crime Stop toll-free number 08600 10111.

Information can also be shared anonymously via the MySAPSApp.

A major breakthrough has been made in a deadly cash in transit heist that shocked Mpumalanga, with a 41 year old suspect arrested in connection with the brutal attack that left security guards dead.

The suspect was arrested at the Springs Magistrates Court on Thursday by members of the Hawks Serious Organised Crime Investigation unit, working together with the Nkangala District Flying Squad.

Police say the arrest is linked to a violent incident that took place on 27 February 2026 in Emalahleni.

According to reports, heavily armed suspects ambushed security guards as they were preparing to load money into an ATM.

During the attack, one security officer was shot and killed on the scene, while another sustained fatal injuries.

The suspects then fled with an undisclosed amount of cash, using a VW Jetta and a VW Polo sedan as getaway vehicles.

Authorities have confirmed that the arrested suspect is expected to appear before the Witbank Magistrates Court on Monday.

He will face multiple serious charges including murder, attempted murder, robbery with aggravating circumstances, conspiracy to commit murder and charges related to illegal immigration status.

The case has once again highlighted the extreme dangers faced by cash in transit security personnel, as well as the ongoing threat posed by organised crime syndicates operating across the country.

Police investigations are continuing, with more arrests expected as authorities close in on other suspects involved in the deadly heist.

Detailed evidence before the Madlanga Commission has shed light on the internal mechanics of how PKTT cases were processed and transferred, revealing a system heavily reliant on manual procedures and fragmented oversight.

Witnesses described a pipeline that depended on physical files, email correspondence, and individual initiative rather than integrated digital systems. Commissioners expressed concern that such reliance increases the risk of delay, miscommunication, and loss of accountability.

According to testimony, PKTT dockets were prepared for transfer through internal verification processes, but once handed over, there was limited ability to track progress centrally. Provincial units maintained their own registers, often using different formats and standards.

Commissioners questioned whether SAPS leadership had a real-time view of where sensitive cases were within the system. Witnesses conceded that no consolidated dashboard or monitoring mechanism existed to provide such oversight.

The commission heard that in some instances, case movement depended on personal follow-ups rather than automated alerts. This meant that proactive supervision was contingent on individual diligence rather than institutional design.

Legal representatives highlighted the risks associated with manual systems in high-stakes investigations, particularly where delays can compromise evidence or witness cooperation. Commissioners noted that modern policing increasingly depends on digital integration, an area where SAPS appears to lag.

Testimony further revealed that responsibility for updating records was not consistently defined, leading to gaps in documentation. In some cases, commissioners were told, it is unclear who last handled certain dockets.

The commission is now probing whether the PKTT pipeline was ever designed with scalability in mind, or whether it evolved reactively in response to operational pressures.

Findings on this issue are expected to inform recommendations on modernising SAPS case management infrastructure.

The Madlanga Commission has heard testimony pointing to a significant breakdown in communication and coordination between the Provincial Key Task Team (PKTT) and provincial detective units following the transfer of sensitive criminal dockets.

Witnesses described a handover process that prioritised administrative compliance over operational continuity. While physical files were transferred and formally acknowledged, little attention was given to ensuring that investigative context, strategic intent, and accumulated intelligence moved with the dockets.

According to evidence, PKTT investigators who had worked on cases for extended periods were not systematically briefed to receiving units. Instead, provincial detectives were often left to reconstruct complex investigations from written records alone, a task made more difficult by inconsistent documentation practices.

Commissioners questioned why no structured debriefing sessions were conducted between outgoing PKTT members and incoming investigators. Witnesses acknowledged that no such protocol existed, and that communication after transfer was largely informal and ad hoc.

The commission heard that this lack of structured engagement resulted in investigative drift. In several cases, lines of inquiry previously identified by PKTT were not pursued with the same urgency or clarity, leading to duplication of work or abandonment of promising leads.

Legal representatives assisting the commission noted that continuity is especially critical in organised crime and corruption cases, where institutional memory plays a central role. Commissioners echoed this concern, warning that investigative resets can significantly weaken cases.

Testimony also revealed that provincial units were sometimes hesitant to seek clarification from PKTT counterparts, citing uncertainty about jurisdiction and authority after transfer. This further deepened the disconnect between units.

The commission is examining whether this breakdown reflects broader structural weaknesses in SAPS inter-unit coordination. At issue is whether the organisation is equipped to manage transitions in complex investigations without losing momentum.

As hearings continue, the commission is expected to consider whether mandatory handover frameworks should be instituted to safeguard continuity in future high-risk cases.
